Plain-English summary
This report describes a rare case of group C streptococcal arthritis, which is an infection in the joints, affecting a healthy 42-year-old horse trainer who likely caught it from a mare. The diagnosis took some time, but after surgery to drain the infection and treatment with penicillin, the trainer improved, although he needed a procedure to move his knee while under general anesthesia. While this type of arthritis is uncommon, it can happen even in people who seem healthy and have no obvious risk factors, especially if they are often around farm animals. In this case, the treatment was successful.
Abstract
Presented is the third known case of a group C streptococcal arthritis, this case documented in a healthy 42-year-old horse trainer, apparently transmitted by a mare. After a delayed diagnosis, the patient responded favorably to surgical drainage and parenteral penicillin G, but required a manipulation of his knee under general anesthesia. Although rare, group C streptococcal arthritis can occur in hosts with no apparent predisposing factors. Frequent exposure to farm animals may increase the risk.
